    We specialize in auto related injuries and injuries occurring in the workplace involving workers compensation. We offer a full service case management program for senior citizens and in addition specialize in adolescent and pediatric cases. 
     
    Rehab Innovations will serve as your advocate and provide you with the respectful care you deserve. We establish relationships between you and your physician to develop a specialized treatment program. We provide registered nurses and licensed social workers to meet your needs.
     
    Are you currently hospitalized? We handle discharge planning. Let us coordinate your recovery and navigate the healthcare system for you. We promise you will have one less worry when we make your medical experience our top priority.
     
    Security of your personal information is important to us. Patient files and documentation are confidentially maintained according to state regulations and other compliance by our trusted team.  
     
    Once we review your case, you will be assigned a case manager. Your case manager will assist you with the following: 
     
    • Scheduling doctor appointments
    • Arranging transportation if you need it, please allow 24 hours notice
    • Attending your doctor appointments with you
    • Maintaining communication and progress notes with your therapists
    • Updating your insurance adjuster and attorney on your progress
    • Ordering any medical equipment needed or prescribed
    • Assisting you in finding handicapped accessible housing if applicable
